2010 Young at Art Field Trip Request Form

General Reservation Guidelines:

Deadline for field trips is April 1, 2010.

Requests will be processed in the order received.

Please submit one reservation form for each class. Classes with reservations will have tickets set aside for them on their specified date.

One adult chaperone is required for every 10 students.

Classes that are performing for the festival do not need to submit a field trip request form.

Classes without a reservation may collect tickets for museum entrance between the hours of 10AM and 1PM on May 11, 12, 13, and 14.

Transportation arrangements will only be made for groups perfoming at the festival. Classes that need transportations arrangements for a field trip must contact the transportation office directly.
